Veteran (returning) varsity athletes are required to complete an “Annual Veteran Sports Physical” with ����ֱ�� Health Services.  If you missed your appointment during the spring semester or are re-joining a team then please call Health Services to schedule an appointment for your “Annual Veteran Sports Physical” at  860-832-1926
Once your appointment is scheduled you will receive the associated annual Health Service forms for you to complete on-line in .   

Please Note: 
To be medically cleared for sport you must have an up-to-date tetanus (valid for the entire upcoming athletic year) and results of your sickle cell test lab results.  It is your responsibility to up-load evidence of both requirements into .
